Home and Community‑Based case management services are centered care services delivered in the home and community. Program services fall into the categories of health services and human services. Primarily responsible for the human service aspect of case management to meet the psychosocial needs of members.
Exempt/Non‑Exempt: Starting wage $22.13 per hour. Actual offered wage is based on applicable experience.
