Qualifications

• CNA/PCA qualifications/credentials as required.
• Experience with personal care and activities of daily living.
• Ability to follow a care plan and report changes or concerns.

Responsibilities

• Personal care: bathing, grooming, dressing, hygiene, toileting and incontinence care.
• Ambulation, transfers and assistance with gait belt when appropriate.
• Meal preparation, companionship and light housekeeping.
• Transportation/errands and medical appointments accompaniment.
• Medication reminders and range-of-motion support as trained.
• Follow care plans and report changes or safety concerns.
