Qualifications

CNA or PCA qualifications/credentials as required.
Personal care and activities-of-daily-living experience
Reliable attendance and clear communication
Respect for client dignity and home routines
Ability to follow a care plan and report changes

Responsibilities

Bathing, grooming, dressing, hygiene, toileting, and incontinence care.
Ambulation, transfers, gait belt, and Hoyer lift assistance as appropriate.
Meal prep, companionship, social engagement, and light housekeeping.
Transportation/errands to medical appointments and errands.
Medication reminders and mobility ROM assistance.
Glucose monitoring, Foley care, hospice-related tasks when trained.
Follow care plan and report changes or safety concerns.
