Qualifications

• CNA or PCA qualifications/credentials as required.
• Personal care and activities‑of‑daily‑living experience required.
• Reliable attendance and clear communication are essential.
• Ability to follow a care plan and report changes or concerns.

Responsibilities

• Personal care: bathing, grooming, dressing, hygiene, toileting, and incontinence care.
• Ambulation, transfers using gait belt or Hoyer lift as appropriate.
• Meal preparation, companionship, social engagement, and light housekeeping.
• Transportation/accompaniment to errands and medical appointments.
• Medication reminders and prescribed range-of-motion assistance.
• Glucose monitoring assistance, Foley care, bed-bound care, hospice support when trained.
• Follow the care plan and report changes or safety concerns.
