Job summary

Action Health Staffing is seeking a Certified Nursing Assistant to provide in-home care in the Williamston area. The role includes direct patient care, helping with personal hygiene, transfers, and mobility, and following the Plan of Care under RN/PT/OT/SLP supervision.

You will keep records, use EVV for tasks, and report changes to the supervising nurse. Reliable, compassionate and privacy-respecting care is essential for this position.

Qualifications

• Must maintain DFS certification.
• Read, write and follow directions.
• Ability to lift and transfer patients safely.

Responsibilities

• Provide direct patient care as assigned.
• Assist with personal hygiene care (bathing, shaving, hair care, mouth care).
• Assist with transfers, repositioning, and ambulation.
• Provide basic nursing care under supervision of RN/PT/OT/SLP.
• Prepare meals with dietary considerations as directed.
• Provide companionship and activities aligned with patient needs.
• Light housekeeping duties as assigned.
• Maintain complete and accurate records per agency.
• Record and report changes in physical or psychosocial condition.
• Complete EVV tasks on aide plan and timesheet when needed.
• Demonstrate dependability by punctual reporting.
• Assist in enabling independence within patient abilities.
• Perform other duties per care plan.
• Call Action Health Staffing with questions and uphold patient privacy.
