Job summary

Monogram Health is seeking a registered nurse to join our care management team. You will build trusting relationships with patients in their homes, develop adaptive care plans with providers, and monitor patient health to improve quality of life for those with chronic kidney disease and related conditions.

The role requires active RN licensure, BLS, and 2+ years in care management. Local travel for visits is frequent, with some training travel to Brentwood, TN.

Qualifications

• Must hold active RN license in the posted state.
• Minimum 2 years of care management and/or CKD/ESRD patient experience.
• BLS certification required; company supports certification during onboarding.
• Willingness to travel locally for in-home visits.
• Strong communication and interpersonal skills with patients and families.

Responsibilities

• Develop and adapt care plans with patients and providers.
• Conduct in-home care management visits and follow care plans.
• Monitor biometric data and intervene per protocol.
• Coordinate with pharmacists and prescribers for medications.
• Educate patients on CKD/ESRD, dialysis, and related comorbidities.
• Engage families and social supports in education and care.
