Qualifications

• Minimum 2 years of relevant work experience in community health or care coordination.
• High school diploma or GED required.
• Valid driver’s license and reliable transportation within the territory.
• Experience in chronic care management or with chronically ill/elderly patients.
• Proficiency with EMR systems and basic computer skills.
• Strong interpersonal communication and patient-centered approach.
• Multilingual skills are a plus.
• Prior home care experience is beneficial.

Responsibilities

• Deliver chronic care management in patients' homes and communities (90% of time) and virtual care (10%).
• Guide patients through enrollment and coordinate with providers to ensure adherence to care plans.
• Build relationships, coordinate care, and educate patients to improve outcomes.
• Engage in outreach, documentation, and collaboration with interdisciplinary teams.
