Job summary

Advocate Support Services, Inc. is seeking a Case Worker for Community Behavioral Health working in the Maryland service areas. The role is full-time and involves substantial field work in clients' homes and communities.

You will deliver Psychiatric Rehabilitation Program (PRP) services to adults living with serious mental illness, with supervision and a supportive team. Responsibilities include meeting clients in their homes, developing IRPs, coordinating with therapists, and maintaining timely

Qualifications

• Valid driver’s license required.
• Reliable personal transportation essential.
• Ability to travel throughout Maryland service areas.
• Prior PRP or behavioral health experience preferred.
• Strong interpersonal and documentation skills.

Responsibilities

• Meet with clients regularly in homes and communities to provide rehabilitation services.
• Develop IRP and coordinate with therapists, clinicians, and families.
• Complete accurate documentation in electronic health records.
• Organize weekly schedule to meet client needs and program expectations.
• Provide skill-building and rehabilitation services in community settings.
