Job summary

Imind Health LLC is seeking a Community Support Worker to provide case management and direct support within PRP and MTS programs in Lanham, MD. The role focuses on aiding individuals with severe mental illness to achieve stability and community integration.

Responsibilities include engagement, home visits when appropriate, and documentation in EHRs. Candidates should have strong communication, de-escalation skills, and a willingness to travel within the service area.

Qualifications

• Experience providing case management, direct support, or behavioral health services to individuals with severe and chronic mental illness.
• Experience with PRP and/or MTS or similar programs is preferred.
• Strong interpersonal, verbal, and written communication skills.
• Ability to stay calm, flexible, and professional in challenging situations.
• Proficiency with basic Microsoft Office and electronic documentation.

Responsibilities

• Provide direct support, case management, and rehabilitation services to program participants.
• Develop and maintain professional relationships with participants to build trust.
• Assist participants with identifying needs, goals, and connecting to services.
• Transport participants to appointments and community activities when needed.
• Assist with accessing social services, housing resources, healthcare, transportation, and other supports.
• Provide services in community settings and homes when appropriate and safe.
• Support participants in developing skills for independence and stability.
• Maintain confidentiality and comply with program requirements.
• Document accurately in electronic health records and systems.
• Participate in team meetings, care planning, and care coordination.
• Engage with community organizations and referral sources to support outcomes.
