Job summary

IMind Health is seeking a Community Support Worker MTS/PRP in Lanham, Maryland. This contract role supports individuals living with severe and chronic mental illness through PRP and MTS programs, delivering case management and rehabilitation services in community settings and clients' homes when appropriate.

Ideal candidates have strong engagement skills, experience with PRP/MTS-like services, and are capable of working independently within a multidisciplinary team.

Qualifications

• Demonstrated experience providing case management, direct support, or behavioral health services to individuals with severe and chronic mental illness.

Responsibilities

• Provide direct support, case management, and rehabilitation services to program participants.
• Develop and maintain positive professional relationships with participants and build rapport.
• Assist participants with identifying needs, setting goals, and connecting to resources and services.
• Accompany or transport participants to appointments and community activities as appropriate.
• Assist with accessing social services, housing, healthcare, transportation, and other supports.
• Provide services in community settings and participants' homes when appropriate and safe.
• Support participants in developing skills for independence and stable community living.
• Utilize de-escalation and crisis-intervention skills as needed.
• Complete timely documentation in electronic health records and systems.
• Maintain confidentiality and comply with requirements.
• Participate in team meetings and care planning.
