CarePlus NJ, Inc. in Bergen County is seeking a Case Manager for Healthy Families. You will provide in-home education and supportive services to new and expectant parents, focusing on families at risk of child abuse and neglect.

You will conduct prenatal and postnatal screenings, complete assessments, and deliver long-term prevention services including case management, home visits, calls, and parent support groups. This role requires Spanish language communication and a valid driver’s license.

Qualifications

• One year of experience in a mental health-related field.
• Must communicate with clients in Spanish.
• Valid driver’s license and must be insurable by the agency's automobile insurance

Responsabilités

• Facilitate psycho-educational services for consumers and their family members on wellness topics.
• Engage in supportive counseling, crisis intervention, and offer on-site as well as outreach services (including transport).
• Communicate observations and information about the consumer in the service record and to team members.
• Participate in the assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery, and at discharge.
• Collaborate with team and providers to establish bio-psycho-social assessments and care plans and participate in quality assurance.
• Provide input into consumer-centered care plans in collaboration with team members.
• Work with administrative staff to ensure services are documented based on billing and regulatory standards; collect consumer information and demographics.
• Depending on scheduling, may occasionally work outside regular hours.
