Job summary

Acenda Integrated Health seeks a Family Support Worker to establish long-term relationships with families served by the Healthy Families program in New Jersey. The role focuses on linking parents to social services, promoting healthy child growth, and coordinating with community resources.

Applicants should have a High School Diploma or GED and at least two years of relevant experience with young children and families. Bilingual proficiency is a plus, and travel across New Jersey is required.

Qualifications

• Minimum of a High School Diploma or GED with two years of experience with children ages 0-3 and families.
• Experience with community agencies and referral processes.
• Ability to travel throughout New Jersey and attend required trainings.
• Must have an acceptable driving record and insured vehicle.

Responsibilities

• Outreach families referred through Central Intake or a Service Provider.
• Use a standardized screening tool to determine eligibility for home visitation services.
• Interview the Parent Survey with parents to identify strengths and risk factors.
• Help increase the family support system and problem solving for the family.
• Assist the family in establishing goals and a plan to achieve them.
• The assessment of the normal growth and development of the target child.
• Identify and refer to other supportive agencies, including healthcare appointments.
