Job summary

Center for Family Services in Woodbury, NJ is seeking a Case Manager for our Maternity Group Home to provide screening, assessments, advocacy and ongoing case management for homeless families transitioning back to the community.

The role emphasizes collaboration, service planning, documentation, and connection to community resources, with responsibilities spanning intake, goal setting, housing stability, and referrals.

Qualifications

• Bachelor's degree in Social Work, Psychology or Human Services is mandatory.
• Excellent communication and organizational skills with basic computer knowledge.
• Excellent phone manners and customer service skills.

Responsibilities

• Provide intake, screening, assessments, shelter housing placement paperwork and ongoing monitoring for candidates.
• Provide participant intake, address immediate needs, assessments, and ongoing goal setting and monitoring.
• Assist participants to establish stable housing by helping them develop problem solving and goal setting skills.
• Assist participants in determining their needs in all areas, including employment, education and financial needs.
• Provide information/referral ensuring warm transfer with involved programs.
