Job summary

Northeast Arc is seeking a MassAbility Case Coordinator to provide direct, in-home case management for individuals with disabilities in the community. You will assist with housing, benefits, transportation, healthcare, and other supports to promote independence.

This full-time role requires strong organization, communication, and problem-solving skills, plus the ability to travel within the service area and work collaboratively with families, providers, and funding agencies.

Qualifications

• Associate's or Bachelor's degree in Human Services, Social Work, Psychology, or related field.
• Experience supporting individuals with disabilities or in human services preferred.
• Strong organizational, communication, and problem-solving skills.

Responsibilities

• Provide individualized support and case coordination for assigned participants.
• Assist with housing, budgeting, public benefits, healthcare appointments, transportation, and community engagement.
• Help participants achieve goals outlined in their service plans.
• Connect individuals with community resources and support networks.
• Maintain accurate documentation and program records.
• Collaborate with participants, families, providers, and funding agencies.
• Participate in an on-call rotation as assigned.
