Job summary

Sharecare, Inc. through CareLinx is seeking experienced in-home caregivers to provide non-clinical support to health plan members and private clients in the El Monte area.

You set your availability in the CareLinx app, and our team matches you with clients that fit your schedule and goals. Onboarded caregivers work with the CarePartner team to find right matches based on location and availability; you can build consistent schedules and stay engaged via the app while maintaining patient privacy

Qualifications

• Minimum 6 months of caregiving experience or relevant education/certification.
• Access to a smartphone capable of running the CareLinx app.
• Ability to communicate in English; bilingual caregivers encouraged to apply.
• Reliable transportation to and from client homes.
• Must pass a background check.

Responsibilities

• Assist clients with activities of daily living (ADLs): companionship, meal prep, mobility support, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping.
• Complete and document all care tasks accurately in the CareLinx mobile app during each shift.
• Monitor and report changes in a client’s condition or self-care abilities to the CareLinx clinical team.
• Submit timesheets on time through the CareLinx app.
• Communicate with your CarePartner regarding any case updates or client changes.
• Ensure patient privacy, dignity, and well-being in a safe and secure environment.
• Maintain client confidentiality at all times.
• Additional shifts like transportation, hospital discharge escort, and Telehealth navigation may be offered.
