Job summary

HOME CARE SNE LLC is hiring a compassionate In Home Caregiver to support clients in their homes across Connecticut. The role focuses on enhancing daily living, safety, and comfort through ADL assistance, meal prep, companionship, and light housekeeping.

The ideal candidate has experience in home health care, a patient-centered approach, and strong communication; CNA/HHA/PCA certifications are a plus. Driver’s license may be required for some clients.

Qualifications

• Prior home health care or similar experience required.
• CPR certification highly desirable.
• Strong communication and documentation skills.

Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
• Provide patient care, including monitoring vital signs and administering medications as per care plans.
• Prepare nutritious meals and assist with meal planning to meet dietary needs.
• Maintain a clean and safe environment by performing light housekeeping tasks.
• Offer companionship and engage clients in social activities to promote mental well-being.
• Observe and report changes in client conditions or behaviors to relevant healthcare professionals.
• Support clients with memory care, including those with Alzheimer’s or dementia.
• Utilize equipment such as Hoyer lifts for safe patient transfers when necessary.
• Ensure compliance with HIPAA regulations while maintaining client confidentiality.
