Job summary

OneWell Health Care in Avondale, PA seeks a dedicated Direct Support Professional (Caregiver/DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings.

You will assist with daily living, meal prep, mobility, health monitoring, and documentation while following ISP plans and HIPAA policies. This is a field-based role requiring flexibility and driving to assignments.

Qualifications

• High school diploma or equivalent.
• Experience in caregiving, home health, or direct support for individuals with IDD is preferred.
• Ability to follow written care plans and documentation guidelines.

Responsibilities

• Provide hands-on, person-centered support per individual care plans.
• Assist with daily living activities, meal prep, mobility and medication assistance as authorized.
• Monitor health, safety, and emotional well-being; promote independence and community participation.
• Maintain accurate, timely documentation and participate in trainings and team meetings.
