Qualifications

• CNA or PCA qualifications/credentials as required
• Personal care and activities-of-daily-living experience
• Reliable attendance and clear communication
• Respect for client dignity, privacy, preferences, and home
• Experience with HD equipment or hospice/bed-bound care is valuable

Responsibilities

• Provide personal care: bathing, grooming, dressing, hygiene and toileting
• Assist with ambulation, transfers, gait belt and Hoyer lift as appropriate
• Prepare meals, offer companionship, assist with light housekeeping
• Transport or accompany to errands and medical appointments
• Provide medication reminders and range-of-motion assistance
• Assist with blood glucose monitoring, Foley care, hospice or bed-bound care as trained
• Follow care plans and report changes or safety concerns
