Qualifications

CNA or PCA qualifications/credentials as required.
Personal care and activities-of-daily-living experience.
Reliable attendance and clear communication.
Respect for client dignity, privacy, preferences, and home.
Ability to follow a care plan and report changes or concerns.
Experience with Hoyer lifts, transfers, Foley care, hospice/bed-bound care, companionship, meal preparation, or light housekeeping is valuable.

Responsibilities

Personal care: bathing, grooming, dressing, hygiene, toileting, and incontinence care.
Ambulation, positioning, transfers, gait belt and Hoyer lift assistance when appropriate.
Meal preparation, companionship, social engagement, and light housekeeping.
Transportation/accompaniment to errands and medical appointments.
Medication reminders and prescribed range-of-motion assistance.
Blood glucose monitoring assistance, Foley care, bed-bound care, and hospice supportive care when trained and appropriate.
Follow the care plan and report changes, concerns, or safety issues.
