Job summary

Qualifications

Responsibilities

• Follow client’s individualized care plan as directed by supervisor.
• Assist with activities of daily living (ADL) including bathing, grooming, meal prep, and transport.
• Provide companionship and emotional support while maintaining client dignity.
• Maintain accurate records and report changes to care management.
• Ensure a safe, clean, and orderly environment for clients.
