Job summary

Qualifications

• Requires CNA/HCA/HHA or NAR credential.
• Minimum six months of industry-related experience.

Responsibilities

• Follow each client's care plan as directed by the In-Home Care Supervisor or Manager.
• Assist with activities of daily living (ADL) including housekeeping, medication assistance, personal care, meals, shopping and transportation, and mobility transfers.
• Communicate client changes or requests to the care management team; advocate within scope of license.
• Maintain accurate client records; document medications, activities, observations, and health changes in a timely manner.
• Ensure a safe, clean, and orderly environment; perform housekeeping and laundry duties as directed.
• Safely transport clients to appointments and activities; assist with ambulation and transfers.
• Adhere to safety protocols and HIPAA/privacy requirements; uphold professionalism and confidentiality.
