PALS OHIO, LLC. is seeking an In-Home Care Specialist in Gallipolis, OH to provide person-centered support under an Individual Service Plan.

You’ll assist with homemaker and personal care, health and medical tasks, meal planning, mobility and transportation, and community participation while collaborating with the interdisciplinary team. Requirements include a High School diploma or GED, a current Ohio driver’s license, ability to pass a DOT exam, and a background check.

Qualifikationen

• Must have at least one year of healthcare experience or experience working with people with disabilities.
• Ability to work as part of a team and communicate effectively with service participants, staff, and supervisors.
• Willingness to complete all required trainings and certifications as applicable.

• Provide transportation and direct supervision as needed.
• Maintain documentation including transportation, billing, and program records.
• Support individuals with daily living activities and skills development according to the Individual Service Plan.
• Collaborate with the interdisciplinary team to ensure quality person-centered services.
• Uphold company guidelines, confidentiality, and people-first language.
