Job summary

Qualifications

Responsibilities

• Participate in ISP meetings when requested by the interdisciplinary team.
• Perform tasks per the ISP and prioritize goals.
• Maintain a clean, safe home environment and be responsive to client and family needs.
• Provide basic personal care and grooming assistance.
• Perform homemaking duties including cooking, cleaning, laundry, and shopping.
