Job summary

LCD Agency Services is seeking a compassionate Home Health Aide / Direct Support Professional (DSP) to deliver person-centered care in clients’ homes and community settings in Cincinnati, OH. You will support daily living activities and promote independence under ISP, ODM, DODD, and ODA guidelines.

You will provide personal care, meal support, light housekeeping, and transportation assistance as needed, while documenting shifts and maintaining confidentiality.

Qualifications

• High school diploma or GED preferred.
• Must be at least 18 years old.
• Must pass required background checks.
• Valid driver’s license preferred.
• Reliable transportation.
• Ability to lift up to 50 pounds.
• Ability to obtain First Aid/CPR certification.
• Ability to complete DODD/ODA training.

Responsibilities

• Provide personal care including bathing, grooming, dressing, toileting, and hygiene.
• Assist with meal planning, preparation, and feeding.
• Complete light housekeeping, laundry, and other tasks.
• Assist with medication reminders (if certified).
• Accompany to medical appointments, community activities, shopping, and outings.
• Provide companionship while encouraging independence and community integration.
• Assist with mobility, transfers, and ambulation using proper lifting techniques.
• Follow ISP, Behavior Support Plans, and Nursing Care Plans.
• Monitor changes in condition and report concerns.
• Maintain HIPAA confidentiality.
• Document all shifts before end of workday.
• Communicate professionally with supervisors and team.
• Maintain a clean, safe environment.
• Attend required trainings and follow procedures.
• Perform other duties as assigned.
