Qualifikationen

• Current California Registered Nurses License and CPR certification are required.
• Minimum of one year experience with frail or elderly population; IV certification preferred.
• Valid driver’s license and reliable transportation are required.

Aufgaben

• Administer medications in clinic (oral, topical, IV, vaccinations) per nursing standards.
• Coordinate comprehensive participant care with interdisciplinary team; participate in IDT and Plans of Care meetings.
• Perform initial, semi-annual, annual, and change-of-condition assessments per CMS guidelines; develop care plans and travel as needed.
• Maintain medical records and complete CESD charting; provide progress notes on health goals.
