Qualifikationen

• Active RN license in state of practice.
• Willing to travel to patients' homes.
• Graduate from an accredited school of nursing.
• Current BLS certification.
• Ability to pass state & federal background checks.
• Professional liability insurance required.

Aufgaben

• Conduct thorough assessments to ensure personalized, comprehensive home care.
• Educate patients and families on health maintenance and disease prevention.
• Provide assistance with ADLs with dignity and respect.
• Coordinate schedule with patients and align with plan of care and preferences.
• Monitor for changes in patient symptoms and report promptly to the team.
• Administer medications and treatments as prescribed by the physician.
• Communicate with Case Managers and healthcare providers for coordinated care.
• Maintain accurate documentation of care in the EMR.
• Offer emotional support and build trust with patients and families.
