Job summary

Optum Home & Community Care, part of UnitedHealth Group, is expanding its HouseCalls program in Texas. As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will perform annual in-home health assessments for Medicare Advantage and other members, without prescribing.

This field-based role emphasizes education, gap closure and collaboration with PCPs. Flexible scheduling, no on-call, no weekends or holidays, with a $10,000 sign-on bonus for external candidates.

Qualifications

• Active ANCC or AANP certification required.
• Active, unrestricted Texas license to practice.
• Driver's license and reliable transportation for home visits.
• Ability to work in varied home environments during assessments.
• Eligibility to practice without Collaborative Agreement where applicable.

Responsibilities

• Conduct comprehensive in-home assessments including history, medications, vitals, and exam.
• Perform screenings and point-of-care testing as appropriate.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to members’ PCPs to address gaps in care.
• Recognize urgent situations and intervene appropriately.
• Educate members on disease processes, medications, and compliance.
• Address social determinants of health and provide referrals as needed.
