Job summary

Optum Home & Community Care is hiring for a HouseCalls Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage members in Texas. This field-based role emphasizes education, gap closure, and coordination with PCPs to improve outcomes.

Enjoy flexible scheduling within 8am–8pm across a 7-day week, with no on-call, weekends, or holidays. A $10,000 sign-on bonus is available for external candidates, with total compensation reflecting base pay plus bonus.

Qualifications

• Active ANCC or AANP certification in relevant primary care specialties.
• Unrestricted Texas licensure or ability to obtain by start date.
• Active driver's license and reliable transportation for home visits.

Responsibilities

• Perform in-home health assessments for Medicare Advantage and other members.
• Identify diagnoses, coordinate care with PCPs, and educate patients.
• Address social determinants of health and provide referrals.
• Document findings and participate in care planning with the team.
