UnitedHealth Group’s Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (NP or PA) to perform in‑home Medicare Advantage and related assessments. You will focus on education, gap closure, and collaboration with primary care providers, without prescribing.

Flexible scheduling with 8am–8pm shifts over a 7‑day week, no on‑call, no weekends, and no holidays. This role supports patients where they live and offers growth within Optum HouseCalls.

Qualifikationen

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care.
• For PAs: NCCPA certification and state licensure.
• Active, unrestricted licensure in the state you will practice, without disciplinary actions.
• Active and unrestricted driver’s license; access to reliable transportation for home visits.
• Ability to navigate varied home environments; sit/stand/kneel as needed.

Aufgaben

• Conduct comprehensive in‑home assessments including history, meds, vitals and exam.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address care gaps.
• Recognize urgent situations and intervene appropriately.
• Educate members on diseases, medications and adherence.
• Address social determinants of health and provide referrals.
