Job summary

Element Care PACE is seeking an Advanced Practice Provider (APP) – NP/PA to join the interdisciplinary team in MA. The APP will plan, implement, and evaluate care plans meeting PACE standards and collaborate with Medical Doctors to provide comprehensive primary care to participants.

The role includes annual/semi-annual examinations, home and facility visits, after-hours coverage, patient education, and adherence to the APP/MD Collaborative Practice Guide. Travel is required.

Qualifications

• MSN or Physician Assistant Studies required.
• Massachusetts NP/PA board certification or licensure required.
• Massachusetts DEA registration required.
• Current CPR certification.
• Minimum 2 years geriatric care experience.

Responsibilities

• Conducts comprehensive physical examinations annually and semi-annually.
• Evaluates participants in homes, ALFs, centers for episodic visits and follow-ups.
• Performs home visits for acute/chronic needs as required.
• Participates in After Hours On-Call Coverage.
• Provides health care teaching to patients, families, and caregivers.
