Job summary

Longevity Health is seeking an Advanced Practice Provider (NP/PA) to deliver primary care to nursing home residents under the Longevity Health Plan Model of Care. The APP will collaborate with primary care teams, facilitate advanced care planning, and participate in nursing facility care team meetings, ensuring goals of care are understood and implemented.

The role emphasizes treating in place, adhering to clinical guidelines, documenting health status, and coordinating with telehealth partners

Qualifications

• Master of Science in Nursing as a Nurse Practitioner or Master’s degree from a nationally accredited program.
• Minimum 6 months experience as NP/PA with geriatric/hospice or palliative care exposure.

Responsibilities

• Provides collaborative primary care to Longevity Health Plan members in nursing facility settings.
• Manages pre-authorization, Part A/B services, medication reviews, and quality gap closure.
• Communicates with residents, families, and care teams to align with goals of care.
• Participates in care planning and supports functioning of the care team in the nursing home environment.
