Job summary

Companions & Homemakers HCA #2052 is seeking a Field Coordinator to ensure clients receive care per company standards. The role involves field visits to clients’ homes to assess client needs and caregiver performance, aiding in optimal caregiver/client matching.

The position requires travel within Connecticut and strong documentation, communication, and scheduling skills to support service quality and client well-being.

Qualifications

• Compassion for elderly clients and dedication to the mission.
• Clear and straightforward communication with clients and staff.
• Ability to fill in for clients and perform required care tasks.
• Comfort with using lifting equipment such as a Hoyer lift.
• Strong observational and time-management skills.
• Ability to adapt to last-minute schedule changes.

Responsibilities

• Fill in for clients with last-minute openings, performing companion, homemaker, or personal care tasks as needed.
• Take on new clients until a suitable match is found.
• Visit new clients within 2 weeks to verify information and caregiver match.
• Conduct home visits as requested by supervisors.
• Record detailed notes from visits and observations for supervisor review.
• Escalate concerns promptly to the supervisor.
