Senior Helpers North West Hartford, CT is seeking a compassionate Case Coordinator (CNA) to join our Client Services and Operations team. This dual-role blends in-home client coordination with office administration to ensure timely assessments and caregiver matches.

The successful candidate will conduct client assessments, recruit and schedule caregivers, maintain records, and liaise with families and healthcare partners to promote safety and independence at home.

Qualifications

• Valid Certified Nursing Assistant (CNA) credential recognized by the State of Connecticut.
• Valid driver’s license and reliable transportation.
• Experience in care-giving, healthcare, home-care or related client-facing environment.
• Strong administrative and organizational skills, including record keeping and scheduling.
• Commitment to confidentiality and person-centered care.

Responsibilities

• Conduct initial in-home client assessments and reassessments with supervision.
• Coordinate caregiver assignments based on client needs, availability and location.
• Maintain accurate client records, care plans and schedules.
• Engage clients and families to align on care expectations and services.
• Support daily office operations including scheduling, data entry and documentation.
• Escalate changes in condition or safety concerns to management.
