Job summary

HouseWorks, LLC is seeking a Field Coordinator to ensure clients receive care per C&H standards and to promote homecare as a viable alternative to institutional care. In the field, you will assess client needs and caregiver performance while visiting clients’ homes to optimize matches.

You will travel within the designated CT service area, perform visits, document observations, escalate concerns, and prepare weekly activity reports for supervision.

• Ability to adapt to last minute schedule changes.

Responsibilities

• Fill in for clients with last minute openings, fulfilling the duties of a companion, homemaker, or personal care assistant depending on the clients’ needs.
• Take on new clients until a suitable match has been made.
• Perform visits to new clients within the first 2 weeks of services to ensure accurate information on the client and caregiver match is going well.
• Perform home visits as requested by supervisors to clients’ homes.
• Thorough documentation of pertinent information obtained during scheduled time with a client, whether in home or otherwise.
• Escalation of any concerns to direct supervisor pertaining to observations or learned information.
