Phoenix Home Care & Hospice seeks a highly organized Care Coordinator to ensure clients receive exceptional care. You will connect clients, families, caregivers, and nurses to deliver the right care at the right time.

This role thrives in a fast-paced environment, emphasizing relationship-building and problem-solving. Responsibilities include coordinating caregiver and nursing schedules, communicating updates, and supporting field staff, while maintaining accurate documentation and driving

Qualifications

• High School Diploma or equivalent required.
• Previous office, scheduling, staffing, or healthcare experience preferred.
• Experience working with elderly, disabled, or medically fragile individuals preferred.
• Strong communication, customer service, and problem-solving skills.
• Ability to multitask and prioritize in a fast-moving environment.
• Professional, positive, and team-oriented attitude.
• Strong organizational and computer skills.

Responsibilities

• Coordinate daily schedules for caregivers and nursing staff.
• Build and maintain strong relationships with clients and families.
