Job summary

Right at Home Hilton Head/Savannah is seeking a Care Coordinator to create extraordinary client and family experiences and manage best-in-home-care plans.

The ideal candidate has prior experience in home care or medical offices, can work independently, communicate clearly, and maintain a calm, multitask-friendly office presence, including on-call duties and a valid driver's license.

Qualifications

• Strong communication and organizational skills are essential.
• Ability to make sound judgments and work independently.
• Experience or familiarity with home care or medical office settings is preferred.
• Valid driver’s license with use of insured automobile is required.

Responsibilities

• Develops and maintains client care plans and communicates with clients, families, care partners, and referral sources.
• Ensures safe, appropriate, and meaningful care is provided by interfacing with clients and families.
• Mentors caregivers and supports their care delivery.
• Maintains a calm, positive demeanor and multitasks in a fast-paced office environment.
• Supports on-call duties outside of normal office hours.

The Care Coordinator isresponsible for creating extraordinary client and family experiences and ultimately manages “best in home care” care plans.

• Develops and maintains the client care plan, creating extraordinary client experiences and communicating with clients, families, care partners and referral sources.

• Plays an integral role in interfacing with clients and families to ensure safe, appropriate and meaningful care is being provided.

• Serves as a mentor to caregivers and supports their care delivery.

• Maintains a calm, positive demeanor at all times and is able to effectively multitask in a high functioning office environment.

• The ideal candidate has prior experience working in the home care industry or medical office setting.

The desired candidate will possess the following:

• High school graduate or equivalent with two years of business experience.

• Able to work independently, demonstrating sound judgment.

• Be available as required for on-call duty outside of normal office hours.

• Have a valid driver’s license and use of insured automobile.
