Phoenix Home Care and Hospice is seeking a highly organized Care Coordinator to ensure our clients receive exceptional care. You will connect clients, families, caregivers, and nurses to deliver care at the right time.

Responsibilities include coordinating caregiver schedules, maintaining relationships with clients, and supporting field staff. The role emphasizes communication, organization, and problem-solving in a fast-paced environment.

Requirements

• High School Diploma or equivalent required
• Previous office, scheduling, staffing, or healthcare experience preferred
• Experience working with elderly, disabled, or medically fragile individuals preferred
• Strong communication, customer service, and problem-solving skills
• Ability to multitask and prioritize in a fast-moving environment
• Professional, positive, and team-oriented attitude
• Strong organizational and computer skills
