Phoenix Home Care & Hospice seeks a highly organized Care Coordinator to ensure our clients receive exceptional care. You will connect clients, families, caregivers, and nurses, coordinating daily schedules and communication to deliver timely, quality care.

In a fast-paced environment, you will build relationships, resolve issues, maintain documentation, and support field staff. This role offers growth, ongoing training, and a mission-driven culture.

Qualifications

• High School Diploma or equivalent required.
• Previous office, scheduling, staffing, or healthcare experience preferred.
• Experience with elderly, disabled, or medically fragile individuals preferred.
• Strong communication, customer service and problem-solving skills.
• Ability to multitask and prioritize in a fast-moving environment.
• Professional, positive, and team-oriented attitude.
• Strong organizational and computer skills.

Responsibilities

• Coordinate daily schedules for caregivers and nursing staff.
• Build and maintain strong relationships with clients and families.
• Communicate schedule changes and care updates promptly.
• Support and assist field staff with day-to-day needs.
• Investigate and resolve client and employee concerns.
• Maintain accurate documentation and reporting.
• Partner with clinical and operational teams to ensure quality care.
