Job summary

Humana is seeking a Care Coach 1 in the Miami area to visit Medicaid members in homes and facilities, assess needs, and develop person-centered care plans. The role involves substantial local travel (75-90%) and coordinating services with providers and payers to promote well-being.

You will work weekdays 8:30am-5:00pm, with a starting salary range around $62k–$66k yearly, plus comprehensive benefits starting day one. Humana emphasizes teamwork and community health impact.

Qualifications

• Bachelor’s Degree with at least 2 years of relevant experience (Health Services, Social Work, Psychology).
• Registered Nurse (RN), licensed in the state of Florida with at least 2 years of relevant experience.
• Licensed Practical Nurse (LPN), licensed in the state of Florida, with at least 4 years of relevant experience.
• Case Worker with a minimum of 6 years of Health Services/Case Management Experience.
• Prior experience with Medicare & Medicaid recipients.
• Intermediate to advanced computer skills and experience with Microsoft Office programs.
• AHCA Fingerprint Required.
• TB screening as part of Humana program.
• Valid state driver’s license and vehicle liability insurance.

Responsibilities

• Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long Term Care Facilities and other care settings – 75-90% local travel.
• Assess and evaluate member's needs to establish a member-specific care plan.
• Ensure members are receiving services in the least restrictive setting to achieve well-being.
• Plan and implement interventions to meet those needs.
• Coordinate services and monitor the care plan against member goals.
• Guide members/families toward resources appropriate for their care.
