Job summary

Care Planning Institute, Inc. is seeking a licensed or experienced in-home assessor to visit elderly clients in the Los Angeles area. You will conduct face-to-face interviews, identify non medical care needs, and complete health care assessment documents in the client’s home.

Assessments typically take 30–60 minutes. You are paid $40/hour for time with the veteran and travel. Flexible schedule; work anytime from 8am to 8pm, Monday through Sunday, with weekends acceptable.

Qualifications

• Experience conducting health care assessments in clients' homes.
• Ability to explain non medical care needs and document responses.
• Flexibility to work 8am–8pm, weekends optional.

Responsibilities

• Perform in-home assessments of elderly clients within a 2-hour radius of Los Angeles, CA.
• Interview clients at their homes and complete health care assessment documents.
• Coordinate with clients on care needs and fill required forms.
