Job summary

Care Planning Institute, Inc. is seeking a part-time in-home assessor to evaluate elderly clients within the Grand Rapids, MI area. The role involves visiting clients at home, discussing non-medical care needs, and documenting findings for care planning.

Flexible hours are available from 8am to 8pm, Monday through Sunday, including weekends. Travel time is paid at $40/hour, and background checks and TB tests are required.

Qualifications

• Perform in-home assessments of elderly clients in their homes within the Grand Rapids, MI area (2 hour drive radius).
• Talk with clients and ask questions about non-medical care needs (bathing, dressing, grooming, light house cleaning, etc).
• Able to manage flexible scheduling and travel time as part of the per diem role.
• Background check is required and TB skin test is required if not recently completed.

• Conduct in-home assessments of elderly clients within the Grand Rapids, MI region.
• Gather information on non-medical care needs and document findings.
• Complete assessments within 7 calendar days of notification; travel time compensated at $40/hour.
