Job summary

Mosaic Life Care is seeking a Home Health Aide to provide compassionate in-home care under supervision. You will observe patients, document care activities, and support the health care team to promote safety and continuity of care in patients' homes.

Requirements include a CNA certification, a valid in-state driver’s license, and BLS within 90 days. Two years of relevant care experience are preferred, with travel between system facilities as needed.

Qualifications

• High School Diploma required.
• 2 years acute care, nursing home, skilled nursing facility experience. Home care experience required per condition of participation in the last 2 years.

Responsibilities

• Provides personal care including baths, oral hygiene, dressing, bathing and meal assistance.
• Record vital signs and report changes to nurse or therapist as directed.
• Assist with transfers, ambulation and range of motion per plan of care.
• Support patient self-administration of medications as directed by clinician.
• Document care and observations accurately and promptly.
• Ensure safety, proper use of equipment and adherence to care procedures.
• Perform additional duties as assigned by supervisor.
