Angels Care Home Health is seeking a Behavioral Health Registered Nurse (BHRN) to provide psychiatric evaluation and therapy to homebound patients. You will coordinate care with the interdisciplinary team, patients, families, and referring agencies to support ongoing treatment.

The ideal candidate will have 1–2 years of nursing experience in psychiatric settings, an active RN license, and reliable transportation.

Qualifikationen

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Must have 1-2 years of nursing experience in psychiatric/hospital/home care settings.
• Current RN license in state of practice.
• Current driver's license.
• CPR certification.
• Reliable transportation with auto insurance.

Aufgaben

• Conduct comprehensive in-home assessments to identify physical, psychiatric, psychosocial, and environmental needs.
• Develop, implement, and update individualized plans of care.
• Provide skilled nursing interventions within scope of practice.
• Coordinate care with physicians, interdisciplinary teams, patients, families, and referral agencies.
• Ensure medication profiles are accurate and up-to-date.
• Complete admissions and discharges in accordance with standards.
• Maintain documentation supporting medical necessity and homebound status.
• Complete visit documentation including SNVs and OASIS within required timeframes.
• Utilize behavioral health assessment tools and evidence-based practices.
• Monitor cases for payer, federal, and state compliance.
