Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse (BHRN) to provide comprehensive psychiatric evaluation and therapy to homebound patients. The BHRN will coordinate care with an interdisciplinary team, patients, families, and referring agencies while ensuring accurate medication profiles and timely documentation.

Required are a nursing degree, an active RN license, CPR, and a valid driver’s license, with 1–2 years in a psychiatric setting preferred.

Qualifications

• Graduate of an accredited nursing program.
• 1-2 years of nursing experience (psychiatric preferred).
• Current RN license in state of practice.
• Current state Driver’s License.
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Conduct in-home assessments to identify physical, psychiatric, psychosocial, and environmental needs.
• Develop and update individualized plans of care.
• Provide behavioral health nursing interventions within scope of practice.
• Coordinate care with physicians, interdisciplinary team, patients, families, and referrals.
• Ensure patient medication profiles are accurate and current.
• Complete admissions and discharges per regulatory standards.
• Maintain proficiency with EMR systems and documentation timelines.
