Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse (BHRN) to provide psychiatric evaluation and therapy to homebound patients. You will coordinate with the interdisciplinary team, patients, families, and referring agencies to ensure comprehensive, continuous care.

The role requires 1–2 years of psychiatric nursing experience, an active RN license, CPR certification, and a valid driver’s license with reliable transportation.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license in the state.
• 1–2 years of nursing experience in psychiatric care preferred.
• CPR certification and Hepatitis consent/declination required.
• Valid driver's license and reliable transportation.
• Proof of auto liability insurance.

Responsibilities

• Conduct comprehensive in-home assessments to identify physical, psychiatric, psychosocial, and environmental needs.
• Develop, implement, and update individualized plans of care for quality and continuity of services.
• Provide skilled nursing interventions related to behavioral health and applicable medical conditions.
• Coordinate care with physicians, interdisciplinary team members, patients, families, and referral agencies.
• Ensure patient medication profiles are accurate and current at all times.
• Complete admissions, resumptions of care, recertifications, and discharges per regulatory standards.
• Demonstrate knowledge of OASIS and document medical necessity and homebound status.
• Complete visit documentation timely, including SNVs and OASIS within 48 hours.
