Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse to provide psychiatric evaluation and therapy to homebound patients. You will coordinate care with the interdisciplinary team, patient/family, and referring agency, while ensuring accurate medication profiles and regulatory documentation.

The role requires RN licensure in Texas, 1–2 years of psychiatric or home health nursing experience, CPR, and a valid driving license with reliable transportation.

Qualifications

• Graduate of an accredited nursing program and RN licensure in the state.
• 1–2 years nursing experience in acute psychiatric or home health settings preferred.
• CPR certification and Hepatitis consent/declination provided.

Responsibilities

• Provide psychiatric evaluation and therapy to homebound patients.
• Coordinate care with interdisciplinary team, patients, and families.
• Maintain accurate medication profiles and documentation per regulatory standards.
• Complete admissions, discharges, and recertifications per standards.
• Utilize behavioral health screening tools and evidence-based practices.
• Supervise Home Health Aides and participate in QA activities.
• Maintain EMR proficiency and timely visit documentation.
