Job summary

Angels Care Home Health in Texas seeks a Behavioral Health Registered Nurse (BHRN) to provide psychiatric care to homebound patients and coordinate with the care team and families. You will conduct in-home assessments, develop care plans, and deliver nursing interventions within scope of practice.

You will document thoroughly, manage medications, and participate in QA activities and on-call duties. A Texas RN license and driving eligibility are required.

Qualifications

• Graduate of an accredited nursing program (Diploma, ADN, or BSN).
• 1–2 years of nursing experience in psychiatric/behavioral health preferred.
• Current RN license in Texas.

Responsibilities

• Conduct in-home assessments to identify physical, psychiatric, psychosocial, and environmental needs.
• Develop, implement, and update individualized plans of care for quality and continuity of services.
• Provide behavioral health nursing interventions within scope of practice.
• Coordinate care with physicians, team members, patients, families, and referral agencies.
• Maintain accurate medication profiles and documentation in EMR.
• Complete admissions, resumptions, and discharges per regulatory standards.
• Demonstrate knowledge of OASIS and ensure documentation supports medical necessity.
• Document visits within required timeframes (SNVs/OASIS).
• Use behavioral health assessment tools and evidence-based practices.
• Monitor cases for payer and regulatory compliance.
• Supervise Home Health Aides as applicable.
• Participate in QA activities and chart audits.
• Maintain EMR proficiency and timely visit syncing.
• Meet productivity targets and participate in meetings and on-call duties.
• Adhere to HIPAA and professional standards.
