Job summary

COPE Health Solutions seeks a full-time Advanced Practice Provider (NP or PA) to deliver autonomous clinical care in beneficiaries' homes across California. Role includes AWVs, post-discharge TOC, and long-term stabilization for high-acuity patients.

The APP will document in HCC coding, participate in interdisciplinary care conferences, and ensure CA compliance with practice acts and guidelines. Hybrid travel within CA and a supportive leadership structure are provided.

Qualifications

Master’s or Doctoral degree from an accredited NP program or ARC-PA accredited PA program.
California licensure requirements: RN/NP or PA license and national certification.
Active prescribing authority and DEA registration for practice in California.
Minimum 2–3 years APP clinical experience, home-based care or geriatrics preferred.

Responsibilities

In-Home Clinical Delivery: conduct in-person assessments in patients' homes or via telehealth in CA region.
Transitions of Care (TOC) management: rapid post-discharge home visits and medication reconciliation.
Complex Care Longitudinal Management: manage high-acuity, frail beneficiaries with individualized care plans.
Perform Medicare AWV in home, conduct cognitive screenings, fall risk assessments, and capture HCC codes.
Collaborate with Care Navigation team and IDT on conferences.
Maintain compliant EHR documentation and adhere to CA practice acts and written guidelines.
