H. T. PROF Group is seeking an Advanced Practice Provider (NP or PA) to join a dedicated primary care team in Apple Valley, California.

This full-time, on-site role supports comprehensive, patient-centered primary care under the Medical Director, with a focus on coordinated treatment plans and continuity of care. Requirements include an MSN or DNP or ARC-PA PA program, California NP/PA license, DEA registration, and at least two years in primary care or outpatient family medicine.

Qualifications

• Education: MSN, DNP, or ARC-PA PA program.
• Active, unrestricted CA NP/PA license.
• Active DEA registration.
• Minimum 2 years in Primary Care or Outpatient Family Medicine.
• Knowledge of primary care, preventative health, and managed care concepts.

Responsibilities

• Perform comprehensive physical examinations, obtain detailed medical histories, diagnose, and manage treatment plans.
• Order diagnostics, interpret results, and refer to specialists to support continuity of care.
• Maintain high clinical standards and understand HCC coding requirements.
• Lead end-of-life and hospice discussions when appropriate.
• Develop relationships across departments and participate in meetings and quality initiatives.
