Job summary

Monogram Health is seeking a full-time Palliative Care Advanced Practice Provider to join our in-home care team serving complex patients, including CKD/ESRD, diabetes, COPD, and CHF. The APP will deliver care in patients' homes with care plans developed in collaboration with a Market Physician Executive and Lead Physician.

The role focuses on symptom management, prognosis education, and advance directives, with collaboration across an interdisciplinary pod and field clinicians to optimize

Qualifications

• Active and unrestricted NP or PA license.
• Board certification for NP (ANCC/AANP) or PA (NCCPA).
• Current and unrestricted DEA certificate.
• Ability to practice autonomously without direct supervision.
• Willingness to travel regionally for in-home and clinic visits.
• Active state licensure with readiness to obtain multi-state licensure.
• 2+ years in a palliative care setting.

Responsibilities

• Deliver in-home palliative care to complex patients.
• Develop individualized care plans with interdisciplinary teams.
• Manage symptoms including pain, pruritus, depression, and insomnia.
• Educate patients and families on prognosis and advance care planning.
• Coordinate care with primary physicians, specialists, and pharmacists.
• Document advance directives and ensure goals of care are met.
